What to include in a referral

  • Patient details, contact number and date of birth
  • Reason for referral and visual acuity
  • Relevant history, medicines and diabetes status
  • Insurance details or self-pay, if known
  • Any optometry findings or OCT images

Cataract

Including complex cases and patients living with dementia.

Medical retina

AMD, diabetic eye disease and retinal vein occlusion.

Uveitis

Inflammatory eye disease, including long-term management.

General and emergency eye problems

Red eye, sudden visual change and second opinions.
